Certificate Holder Statement Letter

Attached is the Certificate Holder Statement Letter. Please fill it out because there is a change in the competency surveillance/maintenance program which was originally to be implemented after 1 year from the date of issuance of the certificate, revised to the 15th month from the date of issuance of the certificate. The form is filled out for each certificate holder participant and sent back (scanned) to us.
